Industrial additives, emulsifiers, preservatives, and artificial sweeteners do not just affect health through calories or nutrients; they restructure the microbiome’s molecular makeup and undermine the integrity of the intestinal barrier.
One of the study’s most critical points concerns the actions of international regulators: under the lens are the EFSA and FDA, which continue to approve these substances based on safety criteria drafted before microbiome science came into the picture.
Indeed, Ianiro emphasizes, «Their impact on the microbiota and the intestinal barrier isn’t a negligible side effect. It is a primary pathogenic mechanism. We’re allowing the industrial restructuring of the human microbiota on a population scale without measuring the consequences».

The convergence of clinical research, nutrition, and environmental protection demands a paradigm shift that can’t be postponed any longer. A 5-Point Roadmap for Ecological Medicine
The study goes beyond criticizing the problem and outlines a five-point action plan. To translate the science into health protection, the authors urge: introducing microbiome literacy in schools, reorienting agricultural subsidies toward fresh food production, and adopting responsible management of antibiotics and gastric protectants to safeguard the gut ecosystem. Economically and research-wise, they propose tax incentives for additives harmful to the microbiota, support for microbiome-based preventive programs, and funding dedicated to longitudinal studies focused on the microbiome–disease link.
These reforms are long overdue, as Gasbarrini puts it: «We urge that additive-safety evaluations include their impact on the microbiota and real-world mixture effects. It’s no longer acceptable to declare a food safe by examining only macronutrients while ignoring what it does to the gut ecosystem» adding that «despite a decade of solid scientific evidence, medicine and policymakers have yet to respond», and concluding that «The era of ecological medicine must begin now—not to generate new science, but to save lives».
