A healthier thymus foresees a longer life and a lower risk of cancer and heart disease in adults. An immune organ long neglected, the thymus could instead offer crucial clues about longevity, with new imaging evidence linking thymic health to survival. This is what a Nature study suggests.
Background
Can the thymus influence mortality, especially cancer and cardiovascular outcomes, in adults? This question underpins a study that, using imaging data and a large patient cohort, sought to examine the thymus’s role in predicting how long people live. The thymus produces T cells, specialized immune cells, which—like many other organs—decline with age through a natural process called thymic involution.
The most entrenched hypothesis in recent years held that the thymus becomes nonfunctional, or at least plays a marginal role, after childhood. Newer evidence, however, indicates that the thymus continues to relate to aging of the immune system and to the development of chronic diseases as people get older.
Against the backdrop of rising global rates of cancer, heart disease, and metabolic syndrome, clarifying the thymus’s role and the mechanisms behind its decline could be essential for shaping future public health. More research will be needed to define how the thymus acts within a broad context of long-term health, but the current study offers some early, intriguing clues.
The Tools
Researchers used imaging analyses and artificial intelligence to launch an observational study across two well-established patient cohorts: the National Lung Screening Trial (NLST) and the Framingham Heart Study (FHS), totaling more than 27,600 participants, all of whom underwent computed tomography (CT). Using these CT images, the researchers built a deep learning (DL) model by employing convolutional neural networks and self-supervised learning to quantify overall thymic health from imaging data, assessed through a model grounded in the organ’s structural features.
This assessment produced a continuous score (scaled 0 to 100 for easier interpretation) that serves as an indirect indicator—derived from imaging—of thymic functional status rather than a direct functional measurement.
The grouping of participants was based on population percentiles into low, mid, and high thymic health categories. The 12-year outcomes for all-cause mortality, lung cancer incidence, and cardiovascular disease were analyzed statistically using Kaplan-Meier survival curves and Cox proportional hazards models, with adjustments for age, sex, smoking status, pack-years, and body mass index (BMI).
A further analysis of thymic health examined associations with metabolic markers, lifestyle factors, and inflammatory proteins, including blood biomarkers such as C-reactive protein, cholesterol, and inflammatory cytokines like interleukin-6.
The Results
Thymic health varied widely among individuals, even among those of similar age, illustrating that immunosenescence is not uniform and that thymic health consistently associates with better survival outcomes. The study found that participants with a healthy thymus had roughly half the odds of dying from all causes compared with those with poorer thymic health over 12 years (13.4% vs. 25.5%), with mortality rates remaining substantial even after accounting for other risk factors such as smoking, age, and medical history. The same association was observed in the Framingham Heart Study, though statistical significance did not hold after full multivariate adjustment, providing a partial but more limited replication across the two cohorts.
In addition, participants with better thymic health showed a lower risk of developing lung cancer and lower cancer-related mortality, with a drop to 3.4% in the high thymic health group versus 5.3% in the low thymic health group. Lung cancer deaths were also nearly halved among those with better thymic function. These associations persisted after adjusting for smoking-related variables, suggesting thymic health could contribute to cancer risk beyond traditional risk factors, though subgroup analyses yielded mixed results for incidence based on smoking status.
Cardiovascular Benefits
A healthy thymus also positively impacted the heart, reducing cardiovascular mortality and risk by up to 63% in the NLST study, with numerically larger but less statistically robust reductions in the Framingham study.
Furthermore, the rates of heart attack and heart failure were lower among those with better thymic health, though some associations weakened after adjusting for age, sex, and smoking, indicating more modest or event-specific effects.
Inflammation, Lifestyle, and Immune Health
Beyond the diseases cited, thymic health appears linked to overall well-being: a compromised thymus could raise mortality from lung, metabolic, and digestive diseases, suggesting an influence on general resilience against multiple illnesses rather than a single condition. In terms of lifestyle, smoking harms the thymus, while better metabolic health supports its function, and chronic inflammation can undermine thymic health: persistently elevated systemic inflammatory markers have been associated with lower thymic scores, underscoring a link between immunosenescence and systemic inflammation tied to a spectrum of chronic diseases.
In conclusion, the study appears to establish thymic health as a crucial determinant of adult health, capable of shaping survival, cancer risk, and cardiovascular outcomes. However, individual differences in thymic health can be substantial, reflecting lifestyle and unique metabolic factors in each person. Based on the evidence, the prospect of leveraging the thymus as a target for disease prevention and longevity-promoting strategies is gaining traction.
Nonetheless, as an observational study, the results do not establish causality, nor is it clear whether reduced thymic health increases disease risk or simply mirrors an underlying health decline. This invites new research to address the study’s limitations—such as data largely drawn from Caucasian populations and from specific cohorts; for example, heavy smokers in the NLST may influence generalizability to broader populations. Yet there is every indication that improving lifestyle, reducing inflammation, and preserving immune function could act synergistically to improve long-term health and quality of life.
